INTEGRATED MENTAL HEALTH SERVICE PROJECT
Integrated care is a means to improving health, aiding recovery and reducing inefficiency and waste (NSW Living Well Report 2014).
Sector Connect has been funded by South Western Sydney Primary Health Network (SWSPHN) to conduct consultations and co-design with key stakeholders to develop a model for a new integrated mental health clinic. The service may be delivered within a general practice, providing wrap around clinical care, care coordination and appropriate referrals for people with severe mental illness. The service will aim to streamline communication between different providers, coordinate an existing, fragmented, system and focus on both the physical and mental health of consumers.
In June 2017, Sector Connect engaged with over 200 key stakeholders, including mental health practitioners, primary health practitioners, other allied health practitioners, government and non-government services, consumers and Carers in consultations and a co-design workshop. Recommendations and feedback received will be used to develop the design of a single agreed upon model for an integrated sub-specialty clinic.
