INTEGRATED MENTAL HEALTH SERVICE PROJECT
Integrated care is a means to improving health, aiding recovery and reducing inefficiency and waste (NSW Living Well Report 2014).
Sector Connect has been funded by South Western Sydney Primary Health Network (SWSPHN) to conduct consultations and co-design with key stakeholders to develop a model for a new integrated mental health clinic. The service will be delivered within a general practice, providing wrap around clinical care, care coordination and appropriate referrals for people with severe mental illness. The service will aim to streamline communication between different providers, coordinate an existing, fragmented, system and focus on both the physical and mental health of clients.
If you are a GP and would like to contribute to consultations and the co-design of a new integrated mental health clinic, please complete the survey below.
The consumer/Carer and NGO service surveys have now closed. For more information or if you still wish to contribute, please be in touch.
